We are happy you chose to register with us and will try to do our best to locate work for you. You will need to provide us with a telephone number and have dependable transportation to and from work. Many of our job assignments go full time after a prearranged period of time with the client company. You will have the opportunity to accept or refuse any assignment we offer you. If you do not accept, your refusal is not held against you. We will have other jobs available.
ATTENDANCE POLICY
Individuals hired by Foothills Staffing need to be dependable. You are to be your work station prepared to do your work at the start of the shift and after all breaks. If you know in advance that you will be absent, please inform Foothills Staffing and your supervisor as soon as possible. You should let your supervisor know within half an hour before the start of your shift of your absence, at which time you should let both know when you will report back to work. We recognize illness, emergencies, and events beyond your control will sometimes occur. our office has a 24 Hour answering machine you must call in to, 828-323-1514, to notify Foothills Staffing.
AVAILABILITY POLICY
If you are not currently on assignment for Foothills Staffing we suggest that you call in every other day to check on work availability. We maintain a log of people calling in for work availability to assist us in matching you with work orders that come in daily. If we do not hear from you we must assume you are not actively seeking employment.
EMPLOYMENT/ DRUG SCREEN POLICY
I agree that I have been informed of the requirements of the work for which I am applying and that the information on this application is correct and complete to the best of my knowledge. I understand that it shall be grounds for immediate dismissal if any of the information contained herein is found to be untrue.

Your worksite employer, Foothills Staffing, has entered into an agreement with Congruity HR, LLC, a professional employer organization, pursuant to North Carolina PEO Act Article 89A. I hereby acknowledge that my employment is “at will”, that I may resign at any time and the company may terminate my employment at any time with or without cause.
Time Card Policy
Time cards MUST be turned in by Monday at 5:00 pm in order for the employee to be paid the following Friday. All time cards MUST be signed by you and an authorized person at the company where you are assigned. If you work at a company that has agreed to send your time card, it is your responsibility to make sure that they have done so by Monday at 5:00pm.

Walk Off / Job Abandonment Policy
I understand that if I leave an assignment or job before the job is complete, my hourly wage will be reduced down to minimum wage. This policy includes leaving a job in the middle of a shift and failure to return to the job within 24 hours advance notification to the client and/or Foothills Staffing. I agree that I will notify Foothills Staffing and/or the client 24 hours prior to the beginning of my shift if I have to leave an assignment.
RULES OF CONDUCT
The following rules have been established for each employee. Failure to comply may result in immediate termination. The following actions are prohibited and in some cases may result in criminal prosecution. If there are any questions and/or concerns, please ask the office manager at Foothills Staffing.
- Excessive tardiness and or absenteeism.
- Possession, consumption or reporting to work under the influence of any intoxicating beverages, illegal substances or narcotics.
- Possession of firearms, fireworks, explosives or any kind of weapon.
- Stealing from the employer, another person or client.
- Instigating fights, gossip and using profane and threatening language.
- Indecent and immoral conduct.
- Willful destruction, damage or defacement of company/client property.
- Falsification of any form, timecard, etc.
- Illegal acts.
- Sexual harassment of co-workers, subordinates or supervisors.
Right to Know and Hazard Communication Policy
The following is an overview of the hazard communications and your right to know about information pertaining to any hazardous chemicals used or stored at any work site to which you may be assigned while working for Foothills Staffing. It is not the intent of this document to provide training on the use, disposal, or storage of any specific chemical or hazardous substance. Instead the purpose of the Foothills Staffing HAZCOM policy is to make you aware that virtually any work site may contain hazardous materials and that the North Carolina Occupational Safety and Health Administration requires that the business utilizing or storing these substances provide proper labeling of this substances as well as Material Safety Data Sheets (MSDS) outlining the materials' hazards and emergency and first aid procedures in the event of exposure. In other words you have the right to be aware of any dangerous or harmful substances in your work environment and the location of information about these substances.
When you initially report to any job assignment, it is important that you become familiar with the MSDS locations at that work site. It is also important that you are aware of the location of the nearest safety shower and eyewash in the event of an accidental exposure. If you are not provided with this information at your job location, please ask your supervisor where the MSDS sheets are located/maintained and the location of the safety showers and eyewashes.
Previous training at another facility does not constitute being trained at the facility where you are presently or will soon be working. This training does not constitute you being trained in hazardous communication. This training is for the sole purpose of providing awareness to Foothills Staffing employees who would be required to work with or around hazardous chemicals.
The goal of The Hazard Communication Standard is to reduce injuries and illnesses that result from the improper use, storage, etc. of chemicals in the workplace.
The first step in the understanding the standard is to know which chemicals it applies to. The OSHA definition of a hazardous chemical is so broad that practically every chemical substance or mixture is included. "Hazardous" chemical means any chemical that is a physical hazard or a health hazard.

Material Safety Data Sheet:
Material Safety Data Sheets must be provided by chemical manufactures and importers to all distributors and employers with the initial shipment of each hazardous chemical. The employer must make MSDS sheets for chemicals in a work area readily accessible to all employees in that are and during each work shift. Each MSDS sheet must be in ENGLISH.
Labels:
The Material Safety Data Sheet is complicated. A few important points that each employee should know is: Physical and chemical characteristics (boiling point, for example):
- Health Hazards (signs and symptoms of short or long term exposure) whether the chemical will or might cause cancer
- Emergency and first aid procedures
Think of the label as an all-important, first visible clue to the relative dangers, safety and health risks of the chemical labels must be legible, in English, and prominently displayed on each and every container that contains this material.
Three elements are required on labels on all containers that will leave the workplace:
- Identity of the hazardous chemical(s)
- Appropriate hazard warnings, physical and health
- Name and address of the chemical manufacturer, importer, or other responsible party
Employers are responsible for ensuring that labels are never defaced or removed. DO NOT REMOVE OR DEFACE ANY LABELS
Type of Exposure:
Inhalation- Many chemicals produce a dust, vapor or a mist that can easily be inhaled. Some particles are so small that they cannot be seen and pose a danger before you can smell them.
Ingestion- Many chemicals are harmful even when only a small amount is swallowed. Serious injury can occur through careless personal habits such as smoking and drinking while working and now washing hands before eating and drinking.
Absorption- Many workplace chemicals enter the body through cuts, scratches and unbroken skin.
Personal Protective Equipment: The use of Personal Protective Equipment is encouraged and in many cases required while handling hazardous chemicals. Material Safety Data Sheets list the Personal Protective Equipment that is suggested and/or required while handling and using chemicals.
HEALTH AND SAFETY RULES
We are committed to providing a safe, accident-free, and healthy work environment for everyone. However, excellent safe and healthy conditions do not occur by chance. They are results of diligent work and careful attention to all company policies by everyone.
Safety demands cooperation on everyone's part. Thus, it is important that communication be kept open at all times between the management and employees. Workers who notice hazards or other safety problems, or feel that they need additional training, must notify their supervisor. Supervisors and management at all levels must address these concerns and take corrective action when warranted.
In order for a health and safety program to be effective, it is vital that it be understood and implemented at all levels from management to all employees. Everyone is obligated to know the safety standards for their area or job, and just as important, to abide by them. It is the duty of all employees to perform their work with maximum regard for the safety of themselves and co-workers. Workers who notice hazards or other safety problems, or feel that they need additional training, must notify their supervisor.
Our safety policies are based on past experience and current standards, and are also an integral part of the company's personnel policies. This means that compliance with the policies is a condition of employment and must be taken seriously. Failure to comply is sufficient grounds for disciplinary action or for termination of employment. I accept responsibility for reading and abiding by its contents. I also accept responsibility for contacting Foothills Staffing if I have any questions, concerns, or need further explanation.
General Workplace Safety Rules
- Report unsafe conditions to your immediate supervisor.
- Promptly report all accidents/injuries, incidents to your immediate supervisor.
- Use eye and face protection where there is danger from flying objects or particles, (such as when grinding, chipping, burning, and welding, etc.) or from hazardous chemical splashes.
- Dress properly. Wear appropriate work clothes, gloves, and shoes or boots. Loose clothing and jewelry shall not be worn.
- Operate machines or other equipment only when all guards and safety devices are in place and in proper operating condition.
- Keep all equipment in safe working condition. Never use defective tools or equipment. Report any defective tools or equipment to immediate supervisor.
- Properly care for and be responsible for all personal protective equipment (PPE). Wear or use any such PPE when required.
- Lockout or tagout or disconnect power on any equipment or machines before any maintenance, unjamming, and adjustments are made.
- Do not leave materials in aisles, walkways, stairways, work areas, or other points of e egress.
- Practice good housekeeping at all times.
- Training on equipment is required prior to unsupervised operation.
- Compliance with all governmental regulations/rules and all company safety rules in the following section are required.
- The following workman's compensation restrictions on all assignments are as follows: No operating of motor vehicles (cars, vans, trucks, etc), no climbing on roofs, ladders or anything over 4ft. No operating of heavy equipment. If Client wants to change your original job assignment you must call Foothills Staffing immediately. This is mandatory for coverage of workman's compensation insurance on you.
- As a Foothills Staffing employee I am aware that I cannot climb on ladders, on roofs or any height, run heavy equipment or drive a vehicle for the client without written consent from Foothills Staffing.
SAFETY RULES
Safety rules have been developed over the years due to accidents that have happened "on the job". Contrary to popular belief, safety rules were not developed to slow down production, inconvenience workers and employers, etc. Safety rules were developed to assist employees in maintaining his/her welfare and health. So he/she can productive as well as enjoy his/her time away from the job. It is important that all safety rules be followed. Failure to do so can cause accident, injury, or even death. Employees who violate safety rules are subject to disciplinary action to include: Warning, Suspension, and/or Termination.
- Obey all safety rules at the work location you are assigned. Be aware that safety rules vary from location to location and it is your responsibility to know the safety rules at YOUR work location. Contact your supervisor for a copy of these rules.
- Personal protective equipment required for the job is to be worn at all times.
- You should wear clothing appropriate to the job. Our staff will describe appropriate attire for the job. In general no loose clothing or loose long hair is permitted. If you have specific questions please see your onsite supervisor.
- Guard and other safety devices are not permitted to be removed and/or disabled. If you are asked to operate machinery with guards or safety devices removed, please report this to Foothills Staffing.
- Obey all safety signs.
- All forms of horseplay are prohibited at the work site.
- Employees must seek help when lifting an item heavier that can normally be lifted or in excess of fifty pounds.
- No alcoholic or illegal drugs are to be bought, sold, stored and/or used on the job and/or job site.
- Possession of and/or use of alcohol or illegal drugs on company property, during work hours, or while operating a company vehicle is prohibited.
- All employees will keep their work area clean and free of hazards. Good housekeeping should be maintained at all times.
- Unless specifically told by us at the time of assignment you are not to work at heights over 10 feet.
- Employees shall not jump off ladders, scaffolds, docks or other heights.
- Report any unsafe conditions to our office immediately, (828)323-1514. If necessary call collect.
- Report any conditions that make you feel unsafe, uncomfortable or threatened. This includes any types of intimidation or harassment. Remember harassment may be emotional, physical or sexual in nature.
- Report any accidents to our office immediately, (828)323-1514. Failure to do so may limit your coverage under state. Worker's compensation law.
I understand I am to report any unsafe condition to Foothills Staffing immediately. I also understand that I am to notify Foothills Staffing if asked to operate any machinery or perform any job for which I am not trained or knowledgeable.
As a Foothills Staffing employee I am aware that I cannot climb on ladders, on roofs or any height, run heavy equipment or drive a vehicle for the client without written consent from Foothills Staffing.
INJURY POLICY
- Report ALL work-related accidents and/or illnesses to the supervisor or person in charge within 24 Hours of occurrence. Failure to notify our office (Foothills Staffing) within 24 hours may delay or inhibit payment of Work's Compensation benefits.
- In the event of an injury-you or your supervisor should notify Foothills Staffing Immediately at 828-323-1514. If the injury is after hours call our company cell phone at 828-446-8704.
- In the event that you need to be seen by a physician, Foothills Staffing will schedule an appointment with a physician. DO NOT USE your family physician and other doctor for work related injuries. DO NOT USE the hospital emergency room unless approved by Foothills Staffing. To assist you in the reporting and treatment of any injuries we have obtained the services of the Congruity HR, LLC.. Foothills Staffing has open communication between the employee and our insurance carrier.
- Post-accident drug testing will be performed on all personnel who sustain a work-related injury. If you require medical treatment this drug testing will be performed in conjunction with your medical attention.If you are retreated by a medical facility, please return the statement of physical capabilities form or a physician's disposition to our office by the next working day/shift.
- For your well-being and to facilitate recovery, Foothills Staffing maintains a prompt return to work/light duty policy. Foothills Staffing work with medical care givers, our insurance carrier and client employers to facilitate the rapid return to work of all injured employees.
FAILURE TO FOLLOW THESE PROCEDURES COULD RESULT IN A NON-COVERED EXPENSE!!!!!!
